The school indemnity form, sent to the whole class.

A school indemnity form is what a parent or guardian signs before a trip, sport or outing: consent for the child to take part, usually with medical details, an emergency contact and the school’s terms. With KuduDoc, the school sends it to the whole class, parents sign on their phones, and each signed form is sealed with the learner’s record.

Parents sign on their phones. You see who is still outstanding and remind them. Each signed form is sealed and kept with the learner’s record.

Send one form to the whole class

From the template to the trip list, in the school’s KuduDoc account.

  1. Choose the form

Start from one of the school templates or upload your own form as Word or PDF. Add the activity, the date, the place, any cost, and the questions: a yes or no for each activity, medical details, emergency contacts, who may fetch the child. Set the reply-by date. Any question you can ask in the intake form behind it works here too.

  2. Add the class

Upload the class list from a spreadsheet: learner, class, and each parent or guardian’s email address or mobile number. Next term, reuse it. Choose whether one parent’s signature is enough or both must sign.

  3. Send

Every parent gets their own link, by email or WhatsApp. The invitation and reminders arrive on WhatsApp; the parent signs on a KuduDoc page in the browser. Where the class list has no email address or number for a parent, print a letter that carries that learner’s own link as a QR code.

  4. Watch who is outstanding

One list per class, with four statuses: Signed, Declined, Opened, not signed and Not opened. Remind the outstanding parents in one step, or let reminders on the days you set go out, outside quiet hours. Parents who have replied hear nothing more. The form closes on the reply-by date, unless you keep it open.

  5. Keep

Each signed form is sealed and filed in the learner’s collection in the Library, by year and class. Before the bus leaves, download the trip list: who may go, and who to call. Health answers appear on it only for the teacher in charge.

What parents see on their phones

  • No app, no cost. The parent opens their link and sees the school’s name and logo first, then signs in or creates a free KuduDoc account to answer.
  • The notice before the questions. Who is asking (the school’s name and address), what the form asks, why, whether answering is voluntary and who at the school sees the answers, as POPIA asks when information is collected (s18). The notice also says how long the answers are kept. Every template carries these fields; the school fills them in.
  • Consent first. The first step: “I am this learner’s parent or guardian, and I consent to the school collecting the information on this form.” Then the questions, then a signature with a finger.
  • A copy to keep. The signed copy arrives by email. With the same free KuduDoc account, the parent saves it to their own Library, under School & family: keep your child’s signed forms at home.
  • No is an answer. “I do not consent” is recorded as clearly as a yes.
  • A change of mind. The parent withdraws consent from the same link, and the school is told straight away (POPIA s11(2)(b)).
  • Paper, if a parent asks. No parent has to sign electronically: the ECT Act forces nobody to use electronic documents (s4(2)(a)). If a parent asks for paper, the office records the reply on the list and attaches a scan to the learner’s record. Keep the signed paper too: a sealed scan does not replace a paper original where one is required.

A child’s information, collected with the parent’s consent

What POPIA says

  1. POPIA bars processing a child’s personal information unless an exception applies (s34).
  2. The first is the prior consent of a competent person, such as a parent or guardian (s35(1)(a)).
  3. Health details such as allergies and medication are special personal information under POPIA (s26).
  4. A school may process them where that is necessary to give a learner special support or make special arrangements for their health (s32(1)(c)).
  5. POPIA puts the burden of proving consent on the responsible party, here the school (s11(2)(a)).

What KuduDoc does

  • The consent step comes first, from the parent or guardian on your class list, before any question.
  • The notice POPIA describes (s18) is part of every template: who is asking, what is collected, why, whether answering is voluntary and who sees it. Each form also says how long the answers are kept.
  • The school exception comes with a duty of confidentiality (s32(2)). In KuduDoc, health answers are marked sensitive: they open only for the staff you name, and appear on the trip list only for the teacher in charge. See how KuduDoc protects the forms.
  • Set how long each set of forms is kept, for example to the end of the school year. When the period ends, KuduDoc deletes them (POPIA s14). See how long to keep school records.
  • KuduDoc handles the forms for the school under a written operator agreement (POPIA s21).
  • A person at KuduDoc reviews each school’s details before its first send to parents: a check against anyone posing as a school to collect children’s information.

Each signed form, sealed with the learner’s record

  • One PDF per reply. The answers, the signature, the date and time, and which parent’s link was used.
  • Sealed when it is signed. KuduDoc seals it with a SHA-256 fingerprint, a certification signature and a timestamp anchor: what a seal records.
  • Ordinary electronic signatures. KuduDoc signatures are ordinary electronic signatures under the ECT Act, recognised for ordinary documents (s13(2)).
  • Checked by whoever you share it with. Share a form with a parent, the trip organiser or the school’s insurer: they can check a signed form on the free public checker. A match means unchanged since sealing: a fact about the file, not about what it says.
  • Found in one search. Search “zoo outing” to see the whole class; open a learner to see every form for the year.
  • Opened and signed, each with its time. The record shows when each link was opened and when it was signed. An opened link is not a read form.

School indemnity form template for South Africa

Five forms for what schools ask parents to sign, from a school trip or excursion to a season of sport, each a starting point in plain English with the POPIA notice built in. Here is what each form covers. Sign in on Professional or Business, each with a 14-day trial, to read it in full and download it as Word or PDF; send it to the parents of all your learners on Business. A starting point, not legal advice.

School trip and outing indemnity and consent

  • The learner and class
  • Consent to take part
  • Medical details, and consent to first aid and emergency treatment if the parent cannot be reached
  • Emergency contacts
  • The terms the school sets for the activity
  • The POPIA notice

  • Sport and extramural participation

One consent for a season, by team, with medical details.

  • Medical information and emergency treatment

What the school may photograph or film, and where it may publish it: the newsletter, the website or social media.

  • Transport and lift-club consent

A province or a school governing body may have its own excursion form: upload it in step one and send it the same way. Looking for something else? See the other consent and indemnity templates.

For schools, crèches, aftercare and clubs

  • Primary and high schools. Trips, camps, sport and photos, for one class or the whole grade.

  • Crèches and ECD centres. Outings, medication consent and who may fetch the child.

  • Aftercare and holiday programmes. The term’s consents in one go.

  • Sports clubs and youth groups. Season consent and medical forms, by team.

Questions about school indemnity forms

See the trip form →

What is a school indemnity form?

A form a parent or guardian signs before a school trip, sport or outing. It usually records consent for the child to take part, medical details, an emergency contact and the terms the school sets for the activity. KuduDoc sends it to the whole class and keeps each signed form with the learner’s record.

How do I get every parent in a class to sign a form?

Upload the class list, choose the form and send it: each parent gets their own link by email or WhatsApp, and signs on a KuduDoc page in their phone’s browser. The outstanding list shows who has signed, who has declined and who has not opened the link. Reminders go only to the parents still outstanding, on the days you set.

Can parents sign a school indemnity form on their phones?

Yes. They open their link, sign in or create a free KuduDoc account, answer the questions and sign with a finger, with no app to install. KuduDoc signatures are ordinary electronic signatures under the ECT Act, recognised for ordinary documents (s13(2)). No parent has to sign electronically (s4(2)(a)): a parent who asks for paper signs on paper, and the school adds the reply to the list.

Is there a school indemnity form template for South Africa?

Yes. KuduDoc’s school trip and outing indemnity and consent form is in the template library on Professional and Business: sign in to read it in full and download it as a Word or PDF file. Sending it to a whole class and tracking the replies is on Business, with a 14-day trial. It is a starting point, not legal advice.

Does a school need a parent’s consent to collect a child’s information?

Consent is one route. POPIA prohibits processing a child’s personal information unless an exception in s35 applies (s34). The first is the prior consent of a competent person, such as a parent or guardian (s35(1)(a)); another is processing necessary for a right or obligation in law (s35(1)(b)). KuduDoc’s form asks for that consent before the parent answers any questions, and the signed form records it.

Are a learner’s allergies and medication special personal information?

Yes. Information about a person’s health is special personal information under POPIA (s26). A school may process it where that is necessary to give a learner special support or make special arrangements for their health, under a duty of confidentiality (s32(1)(c), s32(2)). In KuduDoc, health answers open only for the staff the school names.

Can a parent withdraw consent after signing?

Yes. POPIA lets the person who gave consent withdraw it at any time; withdrawing does not affect the lawfulness of what was done before (s11(2)(b)). The parent withdraws from the same link, the school is told straight away, and the learner’s record shows the date.

Does a signed indemnity form protect the school if something goes wrong?

KuduDoc takes no view on that. Whether an indemnity holds depends on its wording, the facts and the law, so ask the school’s legal adviser. What KuduDoc gives you is a sealed record of exactly what each parent signed, and when.
