KuduDoc Forms

Turn a form into a signed, sealed pack.

Build a mobile-first web form once — onboarding, FICA, mandates. Answers flow into your PDF template, supporting documents attach along the way, and the finished pack routes straight into signing with KuduDoc's usual seal, fingerprint and timestamp.

Personal includes 5 signature sends a month, Family 10. Professional makes sends unlimited at R166/seat, VAT included.

The print-and-scan loop, retired

Ask once, on a phone. Get back a document, not a puzzle.

Half of South African admin is a PDF that gets printed, filled in by hand, photographed at an angle and emailed back. Forms replaces that whole loop with one link.

Build · mobile-first

Forms people can actually finish on a phone.

Big touch targets, one clear question at a time, and conditional fields that only appear when an answer makes them relevant — a sole proprietor never sees the company questions. Send it as a link; it opens in the browser, no app and no account for the person filling it in.

And deliberately: conditional logic sits low on our pricing ladder. The incumbents gate it behind roughly $40–49-per-user-per-month tiers. We think a branching form is table stakes, not an enterprise privilege.

Fill · straight into the PDF

Answers land in the template, exactly where they belong.

Map each question to a field in your PDF template once. Every response then produces a clean, filled document — the engagement letter, the mandate, the debit order authorisation — ready for signature. No retyping, no transcription errors, no deciphering handwriting.

Attach · then sign

Supporting documents ride along. Then everyone signs.

FICA and KYC packs need more than answers — certified ID copies, proof of address, debit-order authorisations. Respondents upload them inside the same flow, and the finished pack routes straight into KuduDoc signing: certification signature, SHA-256 fingerprint and independent timestamp on the sealed result. Identity checks on that pack are in build.

Coming to the Business plan: payment collection at signing* — take the deposit or first payment inside the signing flow, so "signed" and "paid" stop being two separate chases.

The packs your admin actually runs on.

If today's version is "print this, fill it in, scan it back", it belongs here.

Client onboarding Engagement letter, POPIA consent and mandate as one sequenced pack — one link, not five emails.
FICA / KYC packs ID copies and proof of address collected with the answers, not chased for weeks afterwards.
Debit order mandates Banking details straight from the client, into the mandate, into signing — nothing transcribed.
New-employee packs Contract details, tax and banking information gathered before day one, signed in the same flow.
Rental applications Applicant details and supporting documents in order before the lease is even drawn.
POPIA consent Purpose-limited consent captured properly and recorded with the pack it belongs to.
Company onboarding (KYB)* Registration documents, directors' IDs, proof of address, tax clearance and B-BBEE certificate — one sequenced pack, coming to the Business plan.

Questions, answered plainly

Frequently asked questions

No. A form is a link — it opens in the browser, works properly on a phone, and never asks the respondent to create an account or install anything. Only you, the sender, need a KuduDoc account.

They fill your PDF template and become part of the pack — a real document, not a spreadsheet row. KuduDoc is hosted in South Africa and built with POPIA in mind: form responses belong to you and your respondent, and they're never sold or used for advertising.

Yes — that's most of the point. Add upload steps for the supporting documents a pack needs (a certified ID copy, proof of address, a bank statement) and they attach to the same pack as the answers, collected in the same sitting instead of trickling in by email.

Properly signed. The filled pack goes through normal KuduDoc signing — legally recognised under the ECT Act for most everyday documents — and the completed result is sealed with a certification signature, a SHA-256 fingerprint and an OpenTimestamps anchor, checkable by anyone at /verify.

A completed form becomes an ordinary envelope, so it uses your plan's document sends: Personal includes 5 a month, Family (R79/month billed annually, VAT included) includes 10, and Professional (R166/seat, VAT included) makes them unlimited. Business adds API access and webhooks — see the pricing page for the full breakdown.
